Physical Therapist Full Time Rehab Clinic

Revitalize PT and Fitness Clinic is seeking a dedicated and experienced Physical Therapist to join our rehabilitation team at our state-of-the-art facility in Oceanside, California . You will be working onsite in our modern rehabilitation center, equipped with the latest technology and tools to assist in patient evaluations and treatments. Conduct thorough evaluations of patients to assess their physical condition, utilizing a variety of diagnostic tools to determine the extent of disabilities or dysfunctions.

Design and implement personalized treatment plans that align with the individual goals of patients based on their age, medical history, and specific health issues.

Document patient care thoroughly, ensuring compliance with regulatory standards and insurance requirements while maintaining accurate, up-to-date medical records through our Electronic Medical Records (EMR) system.

Collaborate with multidisciplinary teams, including physicians and occupational therapists, to coordinate care and optimize patient outcomes.

Maintain a safe, clean workspace, and regularly check therapy equipment, ensuring everything is functioning properly to guarantee patient safety.

Provide support in training staff and developing educational materials pertaining to treatment modalities and evidence-based practices.

At Revitalize PT and Fitness Clinic, you will work with a diverse range of clients, primarily focusing on those recovering from orthopedic surgeries, sports injuries, and chronic pain conditions. A valid Physical Therapy license in good standing within the state of California.

A National Provider Identifier (NPI) for billing and insurance purposes.

A Bachelor's Degree in Physical Therapy or a related field from an accredited program.

At least 2 years of professional experience in physical therapy, with a focus on orthopedic or sports rehabilitation.

Demonstrated ability to develop tailored rehabilitation programs based on evidence-based practices, suited for patients with varying levels of ability and comorbidities.

Experience collaborating in a multidisciplinary healthcare team to enhance patient-centered care.
