Registered Nurse (RN) - Home Care, Full Time, Second Shift

Frasier is seeking a dedicated Wellness Nurse to support residents in our Assisted Living community. This role ensures high-quality clinical care, fosters strong relationships, and promotes a vibrant, person-centered environment where residents can thrive.
This position follows an 8:00 AM to 4:30 PM shift schedule and requires working every other weekend.

Competitive Pay: $30.00-$46.00 per hour, based on experience and qualifications - plus shift differentials, when applicable: Evening shift differential +$1.75/hour
Overnight shift differential +2.75/hour
Weekend shift differential +1.75/hour

Clinical Care & Health Monitoring (40%)
Oversee medication administration, set-up, and reordering per physician orders.
Manage medical treatments and monitor residents for changes in condition.
Communicate with physicians regarding changes, test results, and care needs.
Act independently in routine and emergency situations; Serve as liaison with physicians and vendors; Care Planning & Documentation (20%)
Coordinate post-admission assessments and care conferences.
Participate in quality improvement initiatives and communicate via shift reports.
Resident & Family Communication (15%)
Communicate effectively with families and physicians.
Respect residents’ cultural and spiritual preferences; deliver personalized care.
Model Eden Alternative principles and treat residents with dignity and kindness.
Ensure regulatory compliance and survey readiness.
Train and oversee Resident Assistants in medication administration.
First Aid/CPR or BLS certification.
3-5 years of experience in assisted living or long-term care; dementia care expertise preferred.
You take initiative and act decisively in routine and emergency situations.
You communicate clearly and compassionately with residents, families, and teams.
