Responsibilities
Administer skilled care to patients requiring intermittent professional services. Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC).

Qualifications

Graduate of an approved school of clinical education

Licensed in the state where they currently practice

At least one year of experience as a licensed professional

Basic demonstrated technology skills, including operation of a mobile device

Education and Experience (Preferred)

Experience as a licensed professional may be deferred with a review and approval from regional leadership

Medicare home health or hospice experience is preferred

Requirements

Valid state driver license

Maintain automobile liability insurance as required by law

Maintain dependable transportation in good working condition

Ability to safely drive an automobile in all types of weather conditions

CPR certification
