Description

Assess patients in both inpatient and outpatient environments and create tailored treatment plans
Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
Collaborate closely with nursing staff, physicians, case managers, and fellow rehabilitation practitioners to secure optimal patient outcomes
Guide and educate patients and their families on therapeutic objectives, discharge planning, and the importance of home exercises
Maintain thorough documentation regarding evaluations, treatments, and patient progress, aligning with facility and compliance standards
Engage in care team meetings and interdisciplinary discussions to facilitate coordinated patient care
Education
~ Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications
State licensure as a Physical Therapist
CPR certification is preferred
Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
Skills
Previous experience preferred but various levels of experience are welcome
