• Contract duration: Shift: Days
• Long-term care facility in Helena, MT

Conduct initial and ongoing physical therapy evaluations for residents to assess mobility, strength, balance, and functional abilities

• Develop and implement individualized treatment plans focused on gait training, transfers, dexterity, ROM, strength, and activity of daily living tasks
• Administer evidence-based PT interventions (therapeutic exercises, manual therapy, gait training, and modalities as appropriate) and progress therapy as resident outcomes evolve
• Monitor resident progress, document outcomes, adjust plans, and communicate with the interdisciplinary care team to optimize care
• Educate residents and families on home exercise programs, safety strategies, fall prevention, and activity modification
• Ensure patient safety, adhere to infection control practices, and maintain accurate charting and compliance with regulatory requirements

Active Physical Therapist license in Montana or eligibility for licensure by endorsement

• Ability to work in a long-term care setting and participate in a day-shift schedule
• Willingness to commit to a 13-week travel assignment
• Strong communication and teamwork skills to collaborate with physicians, nurses, and other therapists
• Prior PT experience in geriatrics or long-term care settings preferred
