Guaranteed Hours: 40 per week
Contract Length: 20 weeks
Requirements:
Experience in providing nutritional care
Ability to conduct patient assessments and develop nutritional care plans
Strong communication skills for interdisciplinary collaboration and patient/family education
Roles & Responsibilities:
Provide comprehensive nutritional care to patients identified with special needs or nutritional risk criteria.
Conduct patient assessments and reassessments to ensure appropriate nutritional care.
Develop and document nutritional plans of care in accordance with policy and practice standards.
Educate patients and their families on nutritional needs and interventions.
Collaborate with interdisciplinary teams to enhance patient care and outcomes.
