Home Care Nurse: LPN - Home Health

Pride Health is hiring a Registered Nurse – Home Health for a contract opportunity supporting a leading healthcare organization in Indianapolis, IN .

Compensation Mileage reimbursement: Pay Range: $53.00–$57.00 per hour

Responsibilities Complete skilled home health nursing visits, including OASIS Start of Care (SOC), recertifications, and discharge assessments.

Perform comprehensive patient assessments, medication management, and wound care in the home setting.

Maintain accurate and timely documentation using Homecare Homebase (HCHB) and other home health EMR systems.

Coordinate care plans with the healthcare team while providing compassionate, patient-centered care.

Qualifications Previous Home Health nursing experience required.

Experience with Homecare Homebase (HCHB) preferred.
