Nurse (Health Care)
Molina Healthcare of Illinois
Molina Healthcare seeks a Registered Nurse to support care management and coordination for LTSS members in Wisconsin. You will work with a multidisciplinary team, develop and monitor care plans, and facilitate waiver services and enrollment with a focus on quality and cost-effective member care.
Expect to travel locally 25-40% to visit members and collaborate with caregivers, physicians, and other health professionals to address medical and psycho-social needs.
