Assignment Details:

• 13 weeks Mon - Fri, 8am - 5pm, 40 hours per week
• On-call every other week (Triage nurse takes calls)
• Weekend RN 30 units HCHB. OASIS and HH required No guarantee

Requirements & Qualifications:
Experience: 2+ years in Home Health
HCHB (Homecare Homebase) & OASIS experience: Required
License: Active RN License in the practicing state
CPR Certification: Required
Driver's License: Required
Transportation: Must have reliable transportation and valid insurance
CHPN Certification: Preferred
Education:
Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program (Required)
Bachelor of Science in Nursing (Preferred)

OASIS - Required
Strong clinical judgment and patient assessment skills
Excellent communication and documentation abilities
