DIRECT SUPPORT PROFESSIONAL (Full-Time & Part-Time Shifts)

REM Community Services , a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities.

Summary

Position type: Non‑exempt, full‑time or part‑time, hourly

Scope of role: Supports 4‑6 individuals in residential settings or up to 10 in periodic programs

Key Responsibilities

Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development

Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living

Services and Supports

Direct Support: Assist with socialization and behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living

Records: Accurately document progress and activity; review records and logs to stay abreast of changes in service plans; maintain confidentiality

Management of Individuals' Assets: Make cost‑effective purchases within the individual's budget and according to their choices

Relationships: Maintain professional relationships with individuals, families, guardians, case managers; greet family members and visitors; implement Sevita Customer Service Standards

Reporting: Report alleged abuse or neglect per internal and external standards; report medical, behavioral and other incidents per Sevita policy and external requirements

Rights: Maintain confidentiality, respect rights of persons served, practice universal precautions, assist individuals in exercising their rights

Health Care

Appointments: Accompany individuals to medical appointments and relay orders to providers as needed

Medications: Accurately administer or support self‑administration of medication; document delivery; report errors; maintain security of controlled medications

General Health Care: Monitor health; document concerns; communicate with nurse or supervisor; call 911 in emergencies; follow health care directives

Medical Supplies and Equipment: Follow procedures for medical equipment; report malfunction or disrepair

Meal Preparation: Prepare or support individuals in preparing food; adhere to planned menus and special diets; monitor mealtime and implement dining plans

Employment Responsibilities

Training: Attend orientation and ongoing training; participate in monthly staff meetings

Employee Scheduling: Work scheduled hours, arrive on time, stay until replacement, notify supervisor when unable to work; fill shifts as required without unapproved overtime

Teamwork: Work collaboratively; value contributions; be courteous; communicate openly; listen; show respect

Workplace Safety: Maintain a safe workplace; comply with safety policies; report hazards; participate in safety training

Maintenance

Vehicles: Transport individuals; drive safely; use safety equipment; report accidents promptly

Housekeeping: Perform daily and seasonal housekeeping; maintain a clean, safe environment; report hazards

Maintenance and Repair: Monitor safety; perform minor repairs; maintain yard; report equipment/facility issues

Safety: Check water temperature when assisting with bathing; participate in drills; protect individuals during emergencies

Other Duties

Perform other duties and activities as required

MPA: Mobile Punch Authorized for timekeeping

Minimum Knowledge and Skills

High school diploma or equivalent (preferred)

Six months experience in human services (preferred)

Must be 18 years of age

Working knowledge of computers

Certificates, Licenses and Registrations

Valid driver's license in good standing

Car registration and vehicle insurance when providing transportation

All state‑required trainings and certifications completed within mandated timeframes

Maintain valid driver's license and meet insurance requirements; be approved to be a driver; comply with MVR policy

Work Environment

Work at the program location and may accompany individuals into the community

Physical Requirements

Heavy work: Exert up to 100 pounds occasionally, 50 pounds frequently, 20 pounds constantly to move objects

Employees must cooperate with the licensee and Department of Human Services staff in inspections, inquiries, or investigations

Must have a valid driver’s license in good standing in New Jersey; if commuting from another state, must have a valid license in that state

All employees hired after 6/15/2018 shall submit to drug testing prior to employment and randomly thereafter

Employees must submit to criminal background checks and be cleared by Department of Human Services; name checked against Central Registry of Offenders Against Individuals with Developmental Disabilities and CARI

Employees must complete and demonstrate proficiency in all required trainings

Shall be at least 18 years of age

Rate of pay: $12/hr
