In this role, you will deliver individualized care to patients in their homes while collaborating with an interdisciplinary team to support recovery, mobility, and overall well-being.
Full-time opportunity
As a Home Health Physical Therapist, you will:
Develop and update individualized treatment plans in collaboration with physicians and care team members.
Deliver skilled physical therapy interventions designed to maximize patient outcomes and independence.
Assess equipment needs and provide recommendations when appropriate.
Participate in care conferences, staff meetings, continuing education, and agency training programs.
Supervise Therapy Assistants and Home Health Aides when appropriate.
Communicate changes in patient condition or additional care needs to leadership and physicians.
Maintain compliance with Medicare regulations, agency standards, and infection-control guidelines.
Graduate of an accredited Physical Therapy program
Current Physical Therapist license in CA
Home health experience preferred
