Key Responsibilities

• Conduct comprehensive evaluations by gathering pertinent patient history, performing specific assessments, and deriving insightful conclusions to formulate individualized treatment plans.
• Clearly articulate evaluation findings and treatment plans to both patients and their families, engaging them in goal-setting processes to ensure alignment and buy-in.
• Demonstrate expertise in implementing treatment protocols, adapting to the patient's evolving health status while integrating best practices reflective of the latest healthcare innovations.
• Collaborate with interdisciplinary teams, recognizing when to document and communicate changes in a patient's condition that may necessitate adjustments in their plan of care.
• Oversee all necessary discharge and follow-up planning, ensuring accurate and timely documentation of care activities, including comprehensive progress notes and discharge evaluations that capture the patient's journey.
• Exhibit effective time management skills by maintaining a full patient schedule while meeting productivity targets and contributing to community outreach and marketing initiatives.
• Engage with departmental goals, actively participating in efforts to comply with accreditation requirements and governmental regulations.

Patient Population / Client Focus

You will primarily work with diverse patient groups, including elderly individuals recovering from surgery, people with chronic conditions, and others requiring rehabilitative support in their own homes. Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.
• A High School Diploma or GED is also required.
• Licensure as mandated by state regulations for physical therapists.
• Current American Heart Association's BLS for Healthcare Providers CPR certification.
• No prior work experience is necessary, making this an excellent opportunity for recent graduates eager to start their careers.

Preferred Experience
