Work Environment

This role will require you to work onsite, delivering therapy services within patients' homes. This setting allows for a more personalized rehabilitation experience and enables you to assess and adapt therapeutic strategies based on an individual's living environment. Key Responsibilities

• Direct physical therapy treatments aimed at preventing disability, relieving pain, and improving muscle function while maintaining the client's highest performance level in accordance with the treatment plan.
• Oversee and mentor Physical Therapy Assistants, ensuring the accurate implementation of therapy programs and adherence to best practices.
• Conduct comprehensive evaluations of clients' physical capabilities using diagnostic tests to inform treatment strategies and goals.
• Assess clients' home environments and provide tailored recommendations to enhance their autonomy and rehabilitation outcomes.
• Administer prescribed therapy services as directed by physicians, including manual and non-manual therapy sessions, traction, and other physical modalities.
• Fit and adjust prosthetic and orthotic devices, providing necessary modifications and recommendations to medical professionals as required.
• Participate actively in interdisciplinary team meetings, discussing care plans and collaborating with other healthcare providers to optimize patient treatment.
• Document patient evaluations, treatment plans, progress notes, and discharge summaries accurately in compliance with regulatory standards.
• Educate clients and their families on treatment protocols and the use of therapeutic devices to ensure continuity of care.
• Contribute to the continuous improvement of clinical practices and help develop new strategies benefiting clients and the organization alike.

Patient Population / Client Focus

You will primarily work with a diverse patient population in their homes, including elderly individuals, post-operative patients, and those with chronic conditions in need of rehabilitation services. Your role as a Physical Therapist is crucial in helping these patients regain independence and improve their quality of life through tailored therapeutic interventions.

Qualifications

• Completion of a Doctorate in Physical Therapy (DPT) program.
• An active and unblemished Physical Therapist license in the state of Virginia.
• Strong clinical skills with a focus on manual therapy techniques and developing effective exercise programs.
• Proficiency in electronic medical record systems, particularly Web PT, to ensure accurate and timely documentation.
• A solid understanding of current physical therapy modalities supported by research and evidence-based practices.

Preferred Experience

• Previous experience in a home healthcare setting is preferred but not mandatory.
• Familiarity with interdisciplinary team dynamics and collaborative care models.
• Experience in providing patient education and family consultations regarding therapeutic processes.

Compensation and Benefits
