CLINICAL STAFF PHARMACIST I/PHARMACY/

Job Description Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. A home infusion clinical pharmacist role is available for direct hire in the Columbus, OH area, offering a Monday through Friday schedule from 8:30 a.m. with participation in an on-call rotation. This position involves preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications, acting as a key member of a multidisciplinary healthcare team dedicated to optimizing patient outcomes.

Prepare specialty pharmaceuticals and IV solutions in compliance with state and federal sterile compounding regulations.

Clinical Monitoring: Assess laboratory values, calculate pharmacokinetic dosing (such as for vancomycin), and monitor Total Parenteral Nutrition (TPN) regimens to adjust therapy as necessary. Work closely with prescribing physicians, home health nurses, intake, referral, reimbursement, nursing, clinical, and pharmacy operations teams to ensure seamless patient care and minimize hospital readmissions.

Regulatory Compliance: Maintain adherence to State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other relevant regulatory standards.

Inventory & Shipping: Manage pharmacy inventory, including refrigerated products, and prepare medications and supplies for safe and timely home delivery.

Minimum of 2 years infusion specialty pharmacy experience

Strong knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations

Familiarity with CareTend pharmacy system
