Clinical Liaison (Fairfax / Loudoun / Arlington County, VA)

The Clinical Liaison serves as the first point of contact for hospitalized patients and their families who may benefit from Ennoble Care's hospice and palliative care services. The Clinical Liaison collaborates closely with physicians, interdisciplinary team members, and acute care partners to develop and initiate individualized, goal-aligned care plans.
Serve as the primary liaison between Ennoble Care and acute care facilities, identifying patients appropriate for hospice or palliative care services.
Initiate care plans and coordinate discharge planning in collaboration with Ennoble Care's hospice and palliative care medical consultants.
Promote Ennoble Care's hospice and palliative care programs within hospitals, skilled nursing facilities, and other referral sources.
Build and maintain strong relationships with facility leadership and staff through education, in-service trainings, and community outreach.
Rotate to other regions or departments as business needs require.
Minimum of one (1) year of experience in a comparable clinical setting or unit.
Graduate of an accredited nursing program as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) required.
Base Salary: $50,000 - $95,000, Plus: Monthly, Quarterly, and Annual Bonuses
