Perform wound care , medication management, injections, and disease monitoring

Coordinate care with physicians and interdisciplinary team members
Document visits accurately in Homecare Homebase
Who We're Looking For Active Minnesota Registered Nurse (RN) license
Experience in home health , hospital, rehabilitation, skilled nursing, or acute care
