Rehab Liaison/As Needed
Texas Rehabilitation Hospital of Plano
Texas Rehabilitation Hospital of Plano seeks a Clinical Liaison to connect patients, families, and referral sources with our inpatient rehabilitation services. In this patient-centered, collaborative role, you will perform clinical assessments, determine rehab eligibility, and coordinate smooth admissions for individuals recovering from stroke, brain or spinal cord injury, trauma, and complex medical conditions. You will educate patients and families, partner with an interdisciplinary rehab team, maintain strong relationships with hospitals and providers, and support exceptional care transitions that help patients regain function and independence.
