RESPONSIBILITIES:

• Primary responsibility is to provide medical management to an established patient panel
• Aid in continuity of care
• Position is supported by Heritage Center for Behavioral and Mental Health, comprised of our behavioral health team of LCSWs, LMSWs, and psychiatric nurse practitioners
• Flexible schedule Monday-Sat with earned admin time that has the potential to be completed remotely
• See a diverse patient panel primarily consisting of adults
• Part-time and full-time scheduling available. We will strongly consider experienced providers seeking a flexible schedule or balance alongside another job

QUALIFICATIONS:

-NY state-licensed

-BE/BC

-Willing to commute to the site clinic location and integrate amongst the patient panel to provide care
