Shift: 5-8 hour shifts, M-F + weekend and holiday rotation
Hours per Shift: 8 hours
Experience: Minimum 1 year in a Medsurg floor in a hospital setting
Home Care Home Base (HCHB) experience
- Home health experience

The Registered Nurse will be responsible for managing home health patients.
The RN will case manage roughly 30 patients per week.
RNs are required to perform start of care, recertifications, and daily visits.
Clinical support will be available 24/7 for emergencies, and from 8 AM to 5 PM for other queries.
The RN will be required to work weekends and participate in holiday rotations as needed.
