FlexStaff is seeking a Registered Nurse (RN) with experience in home care (CHHA)
to work full-time as a Nurse Instructor for our client, a certified home care
Schedule: Monday - Friday 8:30 am - 5:00 pm.

Permanent Position.

In this role you will be taking referrals and processing appropriate home care
Active NYS RN License.
* Experience: Minimum of 2 years of nursing or intake experience in a licensed
home care agency (CHHA).

HomeCare HomeBase (HCHB) or Careport
Triage new referrals and identify urgent clinical needs
Review medical records, hospital summaries, and current medications
Verify and obtain physician orders and prior authorizations as needed

Educate patient/family on home health services, visit expectations, and

Identify and escalate safety risks, infection control issues, or barriers to
care
