• As an LVN, you will provide direct nursing care to residents while supervising and supporting Certified Nursing Assistants to ensure the highest quality of care is delivered every day.

Resident Care

Monitor resident conditions and report changes appropriately

Perform skilled nursing procedures including wound care, tube feedings, catheter care, and other treatments

Collaborate with physicians and interdisciplinary team members

Ensure resident care plans are followed and updated as needed

Leadership & Supervision

Direct and support CNAs and nursing staff during assigned shifts

Provide guidance and mentorship to nursing personnel

Ensure compliance with facility policies and regulatory requirements

Participate in quality assurance and performance improvement initiatives

Maintain accurate and timely nursing documentation

Transcribe physician orders and update medical records

Ensure compliance with HIPAA, infection control, OSHA, and state/federal regulations

Communicate effectively with residents, families, physicians, and coworkers

Participate in shift reports, care conferences, and staff meetings

Help create a positive, respectful, and resident-centered environment

Required: Current, active, and unrestricted California LVN License

Graduate of an accredited LVN program

Excellent communication and customer service abilities

Skilled Nursing Facility (SNF) experience

Long-Term Care experience

Compassionate and resident-focused
