Responsibilities

Perform comprehensive patient assessments, monitor vitals, and manage IV therapies.

Support wound care and infection control in a fast-paced environment.

Participate in multidisciplinary rounds and develop individualized care plans.

Lead discharge planning to ensure safe transitions home.

Educate patients and families about conditions, treatments, pain management, medication safety, and self-care strategies.

Document clearly in the electronic medical record.
