JOB DESCRIPTION :

One of the hospitals is looking for Routine Outpatient Pediatrics in Brick, NJ
Coverage:
Days : 12/28,12/29.12/30.
Time 9am-5 pm, 8 Hours coverage
Need to have NJ Med license, Fed DEA.
Routine Outpatient Pediatrics.

PPD : 18-22

SUBMISSION REQUIREMENTS

Must be Board Certified
Must have NJ license and DEA at time of presentation
Must have clean background and Malpractice history- if file has malpractice must email MSP coordinator for approval before presentation submitted.
Must be Board Certified
Must have NJ license at time of presentation*will accept non licensed candidates with IMLC*
Will accept providers that need to get a NJ Med license, Fed DEA.
Must have clean background and malpractice
Must submit EPA and NPDB with presentation
Must send availability with presentation

AS A REMINDER:

All candidates must have met the minimum shift availability listed on the job.
CV must have all dates in MM/YY to MM/YY format
Work History must be in Sequential order with most recent on top
Any Gaps over 30 days must be explained on the CV
EPA (Please use the attached EPA. Its also in Trio under your Name & then click on Help)
All Licenses, Boards and Certs with expiration dates must be listed on CV
Anyone with malpractice must provide a NPDB
Anyone with legal issues must provide supporting documents
