Home Care Intake Specialist - New York
Rockaway Home Care, a division of Foundation For the Elderly, is seeking a dedicated and compassionate Home Care Intake Specialist to join our Inwood office. As a Home Care Intake Specialist , you will play a critical role in processing patient referrals and ensuring a smooth transition into home care services. As a Home Care Intake Specialist , you will be responsible for receiving and processing referrals from referral sources, contacting patients and families to explain services and gather required information, verifying patient insurance and determining eligibility, and educating patients and families on home care services. You will also work closely with referral sources, MLTC plans, Medicaid assistance resources, and internal teams to ensure each case moves forward properly.
Contact patients and families to explain services and gather required information
Educate patients and families on home care services
Send referrals to MLTC plans for potential enrollment or Medicaid assistance
Ensure Rockaway Home Care receives proper authorization for MLTC cases
Communicate with referral sources, patients, families, and community partners
Help ensure a smooth start-of-care experience for each patient
Minimum 1 year of home care intake experience, preferred but not required
Understanding of MLTC requirements and home care intake processes
Computer-efficient and comfortable learning new systems
Self-motivated and able to work with minimal supervision
Competitive salary range of $22 - $26 HOURLY
