The Patient Advocate serves as a community-based business development representative responsible for generating referrals and increasing enrollment in home care services. The position develops and maintains relationships with healthcare providers, discharge planners, social workers, community organizations, patients, and families to promote the company's services and secure new client admissions. The majority of work is performed in the field through in-person meetings and relationship-building activities at healthcare facilities and community locations.
Essential Duties and Responsibilities Develop and maintain referral relationships with physicians, discharge planners, social workers, nursing facilities, hospitals, and community organizations.
Conduct regular in-person visits to referral sources (nursing homes, doctor’s office, clinics and community organizations) to promote home care services and generate new client referrals.
Identify prospective patients and referral opportunities and convert referrals into admissions.
Meet established referral, admission, and business development goals.
Conduct community outreach and networking activities to increase market awareness and generate new business opportunities.
Track referral activity, admissions, and sales pipeline metrics.
Attend community events, healthcare networking events, and professional meetings to develop referral relationships.
Present and explain home care service offerings to prospective patients, families, and referral sources to facilitate enrollment and admission decisions.
Facilitate the admission process for prospective clients and coordinate the transition from referral to enrollment.
Advocate for patients to ensure appropriate services and authorizations.
Develop and maintain strategic referral relationships that generate ongoing admissions and business growth.
Coordinate with internal teams on referrals and authorizations.
Maintain accurate documentation and records.
Provide updates and reports to leadership.
Performance Expectations Achieve established referral and admission goals.
Maintain an active schedule of field visits to referral sources and prospective clients.
Consistently develop new referral relationships while maintaining existing accounts.
Monitor referral conversion rates and implement strategies to increase admissions.
Qualifications: Knowledge Understanding of home care services, Medicaid-funded programs, and healthcare systems.
Familiarity with patient intake, eligibility screening, and care coordination processes.
Skills Excellent verbal and written communication skills.
Strong interpersonal and relationship-building skills.
Ability to explain complex information clearly and compassionately.
Strong organizational and time-management skills.
Proficiency with CRM or intake systems (Zoho or similar preferred).

Education High school diploma or equivalent required.
Associate’s or Bachelor’s degree in healthcare, social work, human services, or a related field preferred.
Experience Prior experience in healthcare, home care, patient advocacy, care coordination, or community outreach preferred.
Bilingual English/Spanish strongly preferred.
Physical Demands Walking, standing, and sitting across community locations.
Frequent in-person and phone communication.
Occasional lifting up to 15 pounds.
Work Environment Community-based, fast-paced, and patient-centered. This position is primarily performed in the field. The employee customarily and regularly conducts in-person visits to referral sources, healthcare facilities, community organizations, and prospective clients. The majority of work time is spent away from the employer's office developing referral relationships and securing admissions to company services.
Flexible schedule based on patient and partner needs.
Work Location: This is a hybrid position requiring regular in-office attendance (1-2) days per week, subject to business needs.
