Join The Christ Hospital Home Health Care team and make a meaningful impact by providing skilled nursing care to patients in the comfort of their own homes.
As a Home Health LPN, you'll provide one-on-one patient care, support recovery, educate patients and families, and work alongside a collaborative clinical team focused on positive patient outcomes.
Monday-Friday schedule
- Guaranteed base hourly rate
- Uncapped bonus opportunities

- Provide skilled nursing care in patients' homes under physician orders
Follow care plans and goals created by the RN Case Manager
Educate patients and families on disease management and preventative care
Help reduce re-hospitalizations through patient education and proactive care
Current Licensed Practical Nurse license in the state of employment
- Minimum of 1 year of LPN experience in an acute care setting required
- Home health experience preferred
- Valid driver's license and active auto insurance
