Community Liaison - Home Care

Community Home Care LiaisonPosition OverviewInterim HealthCare is seeking a dynamic, relationship-driven Community Home Care Liaison to expand awareness of our private care services and develop strong referral partnerships throughout the community. This role plays a critical part in connecting seniors, individuals with chronic conditions, and families with the personalized in-home care solutions they need to remain safe and independent at home.The ideal candidate is an experienced healthcare marketer with a passion for building relationships, educating referral sources, and driving growth within the private duty home care market.Key ResponsibilitiesDevelop and implement strategic marketing and business development plans to increase private duty care referrals and market share.Establish and maintain strong relationships with key referral sources including: Assisted Living Facilities (ALFs), Independent Living Communities (ILs), Skilled Nursing Facilities (SNFs), Rehabilitation Centers, Senior service organizations, Elder law attorneys, Financial planners and community partnersPromote Interim HealthCare's private duty services including personal care, companion care, respite care, dementia care, and transitional care.Conduct regular sales calls, facility visits, and networking activities within the assigned territory.Deliver educational presentations to community groups, referral partners, and healthcare professionals regarding the benefits of private in-home care.Identify opportunities to expand referral relationships and develop new business partnerships.Track referral activity, conversion rates, and business development outcomes using CRM and company reporting systems.Collaborate with clinical and operational teams to ensure exceptional client experiences and successful service delivery.Represent Interim HealthCare at community events, health fairs, professional associations, and senior-focused programs.Maintain a thorough understanding of private pay home care services, industry trends, competitive positioning, and local market conditions.QualificationsEducation & ExperienceBachelor's degree in Business, Marketing, Healthcare Administration, or related field preferred.Equivalent combination of education and experience will be considered.Minimum of 3 to 5 years of successful sales or marketing experience, preferably in:Private Duty Home CareHome HealthSenior LivingHealthcare ServicesProven experience developing referral networks and cultivating long-term business relationships.Strong knowledge of the senior care continuum and community-based healthcare resources.Skills & CompetenciesExceptional interpersonal and relationship-building skills.Strong presentation and public speaking abilities.Ability to work independently and manage a territory effectively.Results-oriented with a demonstrated history of achieving growth goals.Excellent communication, organizational, and follow-up skills.RequirementsValid Driver's License.Reliable transportation.Ability to travel throughout the assigned territory (80% or greater).Ability to attend occasional evening and community networking events.What Success Looks LikeGrowth in private duty referrals and client admissions.Increased visibility and market presence within the senior care community.Strong referral partnerships and account retention.Achievement of monthly and annual business development goals.Exceptional customer and referral source satisfaction.Preferred Experience KeywordsPrivate Duty Home Care, Private Pay Services, Personal Care, Companion Care, Senior Care, Caregiver Services, Aging in Place, Home Care Marketing, Healthcare Sales, Referral Development, Business Development, Territory Management, Community Outreach, Senior Living, Assisted Living, Independent Living, Skilled Nursing, Memory Care, Dementia Care, Hospital Discharge Planning, Relationship Management.Benefits:401(k)401(k) matchingDental insuranceHealth insurancePaid time offVision insuranceExperience:Marketing: 2 years (Required)Healthcare: 1 year (Preferred)Willingness to travel:75% (Preferred)Work Location: In person
