365 Healthcare is recruiting a Board Certified Family Medicine Physician for a full-time permanent outpatient opportunity in Saratoga Springs, New York.

Sign-On Bonus: Up to $75,000
Retention Bonus: $25,000 (Year 2)
Employment Type: Full-time, permanent
Flexible 4- or 5-day schedule
No in-office weekend hours
Phone call only
RN triage handles first contact

~ Telemedicine coverage:
~2-3 hours on a weekend once every 3 months

Established outpatient primary care practice
Physician-led team-based care model
Collaborative support with APPs
Epic EMR
Strong administrative and clinical support staff
Hospital medicine
Leadership or administrative roles
Teaching medical and NP/PA students
Provide comprehensive primary care to patients of all ages
Perform routine exams and preventive care
Collaborate with APPs and care teams
Maintain accurate documentation in Epic
Deliver patient-centered, evidence-based care
Board Certified or Board Eligible in Family Medicine
Strong interest in full-spectrum primary care
Up to $75K sign-on bonus
