Provide home-basedphysical therapy topatientswho have lost mobility, strength and functionality due to an illness or injury
Workas partofahome health teamwhich mayinclude anRN, LPN, CNA, HHA,OTandSLP, focused on the patient’s plan of careand goals
Observemovements,conduct assessments,documentprogressandmodifycare plan as neededAssistpatient withexercises and hands-on therapyto manage pain and increase mobility
Educate patientson their plan of care,establishedgoals, exercises and self-care
Coach family ontheplan of care, therapy goals and safelyassistingwith exercises
A few must-havesfor Home HealthPhysical Therapists:
Graduate ofanaccreditedPhysicalTherapistProgramand activePTlicense in Michigan(NEW GRADS WELCOME!)
1 year of clinical experience, preferably within home health in the last 5 years
CPR certification
Good clinical judgement, strong interpersonal skills,dexterityand compassion
