Compensation

Full-Time Salary:

$90,000 – $110,000 Base Salary + Benefits

Paid-Per-Visit Option:

$65 – $80 per visit

What You’ll Do

As a Home Care Physical Therapist, you’ll work directly with patients in their homes and communities to help them regain mobility, improve function, and maintain independence.

Responsibilities include:

Evaluate patients and develop individualized treatment plans

Deliver skilled physical therapy services in the home setting

Build meaningful patient relationships through one‑on‑one care

Complete documentation accurately and on time

Coordinate schedules and communicate with the clinical support team

Drive clinical initiatives focused on delivering exceptional patient outcomes

What You'll Bring

Qualifications

Active New Jersey Physical Therapist License

Medicare documentation and billing experience preferred

Strong communication and time management skills

Comfortable working independently

Computer literacy

Current CPR Certification

Professional liability insurance

Ability to wear PPE as needed

Valid Driver's License
