TravelNurseSource is working with TalentBurst, Inc. to find a qualified Med/Surg / Tele RN in Sheboygan, Wisconsin, 53081!
Responsible for providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies and procedures. Uses the nursing process to assess the needs of the patient, plan and implement individualized interventions and evaluate the effectiveness of the plan of care. Includes the patient and family in planning and implementing care and develops and maintains a therapeutic relationship throughout the healthcare continuum.

Anticipates and recognizes significant changes in the patient's condition, status, and need for care. Anticipates and alters the plan of care as needed, mobilizes appropriate resources, and collaborates with other health care team members to coordinate patient centered care.
Implements strategies to reduce patient risk and increase patient safety. Applies evidence-based practice to deliver patient care. Consistently incorporates nursing and regulatory standards of care into practice.
Provides ongoing evaluation of patient, team, and unit outcomes and is empowered to take appropriate action when changes are needed. Promotes collaboration among healthcare team members. Assumes a leadership role in the care team and delegates appropriately to unlicensed personnel.
Seeks to identify potential safety issues and assists in the implementation of corrective actions.
Participates as an effective member of the patient care team to formulate an integrated, unbiased, individualized approach to care.
Participates in quality/process improvement initiatives to improve the functions of the department and the patient care experience.
Participates in professional activities which contribute to personal professional development and the development of others. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration, and/or Certification Required:
Registered Nurse license issued by the state in which the team member practices, and
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA).
Completion of an accredited or approved program in Nursing.

Typically requires 1 year of experience in clinical nursing.

Demonstrates progression to provide holistic patient care and the ability to view clinical situations from a patient and family focus. Proficient computer skills, use of electronic mail and electronic medical records systems.
Demonstrates human relation skills with the ability to develop rapport and maintain positive, professional relationships with a variety of patients, team members and physicians. Demonstrates coordination, collaboration, delegation and team building skills.
Team members will maintain annual educational requirements for hospital basedcertifications (e.g. Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.

About Facility:
o 3400 Union Ave. o Level IV Trauma Center
Unit Information:
o Ortho – total hip and knee replacements, spinal surgeries
o General MedSurg – CHF, COPD, respiratory/kidney/liver/GI issues, etc.

• o Nights: 1:5
• o PIV starts, wound care, dressing changes, wound vacs, drains, chest tubes, stable trach management, knowledge of patients on bipap/cpap

o CNA: 1:5-10 ratio depending on shift. o HUC here on days, part time on Eves
o Charge nurse – takes modified patient assignment
o Phlebotomy for peripheral lab draws
o IV/VAT Team – no, notify House Super for hard sticks
o Hospitalist 24/7
o OT/PT/ST during the weekdays
● Technology/Equipment:
o EMR: EPIC – preferred experience
o Could float within scope of practice at the hospital, mostly to MS1, Cardiac MS2, and ED Holding
● Orientation (facility and unit) :
o 1-2 12h shift on the unit
● o Weekend rotation: every third
o Holiday Expectations: Depending on time of year, could work up to 3 holidays in contract

o Shift times: 0700-1900, 1500-0300, 1900-0700, 0300-1500
