Home Health Aide / Direct Support Professional (DSP)

Position Title: Home Health Aide / Direct Support Professional (DSP)

Employment Type: Full-Time and Part-Time Available

The Home Health Aide/Direct Support Professional (DSP) provides compassionate, person-centered support to individuals receiving homemaker personal care and community-based services. The DSP assists individuals in maintaining their independence while promoting dignity, choice, safety, and inclusion within their homes and communities. Services are provided in accordance with each individual's Individual Service Plan (ISP), Ohio Department of Medicaid (ODM), Ohio Department of Developmental Disabilities (DODD), Ohio Department of Aging (ODA), and agency policies.

Provide personal care assistance, including bathing, grooming, dressing, toileting, and hygiene.

Assist with meal planning, preparation, and feeding as needed.

Complete light housekeeping, laundry, and other household tasks.

Accompany individuals to medical appointments, community activities, shopping, and recreational outings.

Provide companionship while encouraging independence and community integration.

Follow all Individual Service Plans (ISP), Behavior Support Plans, and Nursing Care Plans.

Monitor for changes in an individual's physical or emotional condition and report concerns immediately.

Complete accurate documentation for every shift before the end of the workday.

Communicate professionally with supervisors, families, support coordinators, nurses, and other members of the individual's team.

Maintain a clean, safe, and healthy environment for each individual served.

Follow all agency policies, emergency procedures, and incident reporting requirements.

High school diploma or GED preferred.

Previous caregiving, home health, or direct support experience preferred but not required.

Ability to become certified in First Aid/CPR.

Ability to complete all required DODD and/or ODA training requirements.

Understanding of person-centered care principles.

Ability to respond appropriately during emergencies.

Ability to work in various home and community environments.
