Participate in developing individualized Plans of Care. Identify barriers to care and recommend appropriate interventions. Connect patients and families with valuable community resources. Assist with financial assistance applications and long-term planning. Educate and support patients and caregivers throughout their care journey. Provide counseling related to adjustment, coping, loss, and grief. Collaborate with physicians, nurses, therapists, and the interdisciplinary care team. Complete accurate clinical documentation in accordance with agency, Medicare, and federal guidelines. Observe, document, and communicate changes in patient condition. Attend required training and agency meetings.
