Licensed Practical Nurse

LPN will provide nursing service for all assigned participants.

Essential Duties:

• Complete State's Wellness Assessment for assigned participants at state required intervals including demographic information, recording of vital signs, significant health history, personal history, physicians' orders, risk assessments including completion of the medical/health section of the State's Risk Assessment Tool, examinations and evaluations.
• Develop, oversee and maintain assigned participants Wellness Coordination plan.
• Train Direct Support Professionals as needed on physician orders, wellness needs and provide updated information about the participants wellness/health needs.
• Consult with each participant's health care providers as needed.
• Provide weekly consultations and reviews for all assigned participants. Provide face to face visits to each assigned participant as appropriate for the Wellness Tier.
• Communicate with RN any wellness/risk changes that require updating in participants Wellness/Risk Plan.
• Visit SFC participants monthly at the Family Caregiving site. Record vital signs. Review all medications and MAR.
• Advise employees on proper nursing care and potential adverse reactions, interactions, allergies of prescribed medications. (as assigned)
• Complete an examination for any assigned participant on a quarterly basis. Make referrals to physician as needed. (as assigned). Complete quarterly health assessment.
• Train assigned group home participants and staff as needed in areas of how to care for health needs/conditions, personal hygiene, health maintenance, and disease prevention.
• Promote health and wellness and take steps to address problems before they become serious.
• Review all Medical Information Forms MIF's for changes in physicians' orders. Ensure all changes are implemented.
• Will assist in maintaining the EMAR.
• Provide health care coordination to participants that do not have Wellness Coordination and visits to evaluate each group home individual's physical status as needed.
• Provide direct nursing care when appropriate.
• Teach Core A, Core B and other Health Services classes (as assigned).
• Assist with lifting, turning, moving, positioning and transporting participants into and out of beds, chairs, wheelchairs, lifts, etc. (as needed).
• Carry designated phone to provide continuous after-hours phone support to direct care employees. Rotating basis.
• Back-up for Pharmacy Specialist.

Maintain, review, and update all dietary documents.

Will work under the direction of the RN or consulting physician for clinical/medical matters.

Be able to frequently lift 75 pounds independently and over 75 pounds with assistance.

Administer medications as directed.

Model and maintain professionalism, confidentiality, and teamwork at all times, including when dealing with individuals from outside agencies.

Provide and maintain open lines of communication, and good relationships, with all employees at assigned facilities and administrative support employees through phone, e-mail, memos or other professional means.

Perform duties as required to meet state regulations in the discharge of duties.

All other duties deemed necessary.

Requirements:

• Maintain reliable transportation available for use during working hours, valid driver's license and provide proof of automobile liability coverage.
• Maintain flexible schedule to meet the demands of the department.

• Establish effective working relationships.
• Work independently and in a team environment.
• Be proficient in Windows and Microsoft Office products (Computer literate).

Education and/or Experience:

Licensed Practical Nurse with current Indiana license is required. At least one-year experience working with individuals with developmental disabilities is required.

Certifications/Licenses/Registrations:

Current LPN License

Medication Administration: Core A and Core B

CPR/First Aid

The employee must be able to perform the essential duties of this job.

Work Environment:

While performing the duties of this job, the employee is occasionally exposed to fumes, airborne particles and outside weather conditions. The noise level is usually moderate but occasionally loud. Use of Personal Protective Equipment such as vinyl or latex gloves is occasionally required.
