From comprehensive training to providing necessary protective equipment, we're committed to creating a secure environment for you as you deliver exceptional care in our clients' homes.National pre-taxed mileage reimbursementMeetings and charting conducted via supplied laptopFull benefits through St Peters Health partnersPosition Responsibilities:Provide skilled nursing care to hospice patients in the home setting.Monitor patient status and communicate changes to the interdisciplinary team.Support patients and families through education, symptom management, and compassionate care.Assist with implementation of the hospice plan of care.Collaborate with nurses, physicians, social workers, chaplains, home health aides, and other hospice team members to ensure quality care.Qualifications Required:Current New York State Licensed Practical Nurse (LPN) license.1 Year LPN experience1 or 2 years of homecare and/or Hospice experience preferredValid driver's license and reliable transportation.Strong communication, documentation, and organizational skills.No hospice experience? No problem. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
