We are seeking a motivated Physical Therapist (PT) to join our team on a PRN basis . If you're passionate about helping patients regain mobility and independence, we'd love to connect with you.

Position Summary

As a Physical Therapist (PT) , you will evaluate and treat residents to improve mobility, function, and overall quality of life. You will play a key role in helping patients regain independence and achieve personalized therapy goals.

Key Responsibilities

• Perform patient evaluations and develop individualized treatment plans
• Deliver therapeutic interventions to improve strength, balance, and mobility
• Monitor patient progress and adjust treatment plans as needed
• Maintain accurate and timely documentation
• Collaborate with interdisciplinary teams to support patient outcomes
• Educate residents and families on therapy techniques and recovery goals

Qualifications

• Bachelor's degree required ; advanced degree preferred
• Active, unencumbered Physical Therapist license in South Carolina
• Experience in a long-term care or rehab setting preferred
• Familiarity with PointClickCare (PCC) and Casamba preferred
• Strong communication and clinical skills
• Ability to work independently and as part of a team
