Senior Clinical Transformation Manager - Central / Mountain Time Zones

Senior Clinical Transformation Manager (Senior CTM) Responsible for the ongoing clinical management of physician practices participating in UnitedHealthcare's (UHC) Accountable Care Organizations (ACO), Value Based Programs, Patient Centered Medical Homes (PCMH), or Population Health Management programs such as Patient Centered Care Model. The Senior CTM will lead multi‑disciplined practice transformation and practice performance improvement initiatives, guiding practices to achieve targeted goals of improved quality, efficiency, and utilization. The Senior CTM is accountable for driving practice progress toward desired transformational change and performance improvement, while meeting savings goals set for the practice. The Senior CTM reports directly to the Regional Director and mentors other Transformation Managers on the team. As the single clinical point of contact, the Senior CTM communicates practice‑level progress to senior leadership within UnitedHealthcare and the practice. Primary Responsibilities Accountable for successful deployment of UHC's Clinical Support program at the practice level, including introducing and educating practices on the value/use of reporting tools, patient registries and delivery of reports via UHC's Physician Portal. Build and maintain relationships with practice leadership and key clinical influencers involved in practice transformation. Facilitate efficient, effective practice improvement meetings to monitor and discuss progress on the transformation action plan and achievement of milestones. Develop strategies based on performance analysis for improvement, setting specific outcomes and metrics to monitor progress and recommend improvements. Design practice transformation action plans and implement performance‑improvement initiatives to achieve contractually required milestones. Monitor and review progress of the practice in milestone achievement and ensure accountability for successful completion. Use data to analyze key costs, utilize quality data, and interpret results to assess practice performance. Use data to analyze trends and work with stakeholders to agree on and implement proactive strategies, measuring impact using a PDSA rapid‑cycle improvement approach. Assist practices in creating workflows to optimize care delivery, introduce best‑practice improvements, and evaluate outcomes with rapid‑cycle PDSA methods. Educate and deploy technology tools to support Practice Transformation. Integrate technology tools into practice workflows. Build and maintain relationships with UHC Clinical organization members, Medical Directors, local Network leads, Health Care Economic Analysts, and Clinical Analysts to support the program. Consult and partner with internal UHC matrix partners and the practice to identify organizational and structural challenges hindering program outcomes. Collaborate with UnitedHealthcare teams—including Care Coordinators, quality management teams, hospital clinical teams, and behavioral health teams—to support integrated PCP‑driven care for members. Consult and guide the practice on developing innovative solutions, connecting high‑performing practices to spur innovation. Work with practice and Optum to align and integrate Optum care/disease‑management programs and population health services to augment the practice's capabilities. Assist and support department leaders in summarizing and disseminating learning through team updates, written reports, articles, or presentations as requested by directors. Required Qualifications 5+ combined years of experience improving clinical quality, healthcare analytics, or driving clinical transformation initiatives with population health programs, Patient Centered Medical Homes, or ACOs. 2+ years of experience interpreting and utilizing clinical data analytics, outcome measurement, and applying data to drive change. Demonstrated experience leading groups and presenting. Population health management experience within an ACO or health system setting. Proficiency in Microsoft Office Word, Excel, Outlook, and PowerPoint. High‑speed internet access for home office setup. Willingness to travel up to 25% of the time for face‑to‑face meetings. Willing and able to work CDT or MDT hours. Reliable transportation that enables travel to client or patient sites as required. Preferred Qualifications Active, unrestricted RN license in the state of residence. Physician practice experience. Experience with Medicare, Medicaid, and/or commercial payers. Experience with clinical quality, efficiency, or utilization improvement. Experience using a PDSA rapid‑cycle improvement approach. Experience building relationships and presenting to senior level clinical or executive leadership to drive goal alignment. Experience implementing clinical practice transformation initiatives designed to help providers or hospital groups achieve large‑scale health transformation goals. Benefits & Salary Pay is based on local labor markets, education, work experience, certifications, and other factors. The salary for this role ranges from $91,700 to $163,700 annually for full‑time employment. In addition to the base salary, a comprehensive benefits package, incentive and recognition programs, equity stock purchase options, and 401(k) contributions are offered, subject to eligibility requirements.
