Work Environment

This full-time position is primarily home care-based, allowing you to work closely with patients in their own environments. Key Responsibilities

• Conduct thorough assessments of patients' physical conditions and functional capabilities to identify their specific needs.
• Develop and implement personalized treatment plans that incorporate hands-on therapy, therapeutic exercises, and patient education to enhance recovery.
• Utilize state-of-the-art techniques and equipment to foster optimal patient rehabilitation and improve functionality.
• Monitor and document patients' progress using CureMD EMR to ensure transparent communication with all healthcare team members.
• Collaborate with physicians, occupational therapists, dietitians, and other allied health professionals to create a comprehensive care plan that addresses all aspects of patient wellness.

Patient Population / Client Focus

The ideal candidate will work primarily with adult patients aged 40 to 65, each facing diverse challenges that require tailored rehabilitation strategies. You will be instrumental in treating conditions such as orthopedic injuries, post-surgical recovery, and chronic pain management. Understanding the individual circumstances of each patient will be a key factor in delivering effective therapy and achieving successful outcomes.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program.
• An active or eligible Physical Therapy License in the state of New Jersey.
• Strong interpersonal skills with the ability to establish rapport quickly with patients and their families.
• Commitment to continuous professional development and adapting to advancements in physical therapy.

Preferred Experience

• At least two years of clinical experience in outpatient settings, specifically home care, is preferred but not mandatory.
• Previous experience working with patients with similar demographic profiles and conditions.
• Familiarity with electronic medical record systems, particularly CureMD, is advantageous.

Compensation and Benefits

We offer a competitive annual salary in the range of $110,000 to $130,000, depending on experience and qualifications, along with a comprehensive benefits package that includes:
