Carease Health - NY is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a Nurse Practitioner at Carease Health, you will play a vital role in our comprehensive preventative care program, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our patient care approach.

Responsibilities:

• Provide high-quality, patient-centered care to residents in skilled nursing facilities

• Conduct on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation

• Build strong relationships with facility staff and maintain a patient-centered approach

• Collaborate with healthcare teams to provide proactive, preventive care measures

• Participate in quality improvement initiatives to enhance patient outcomes and facility ratings

Requirements:

• Active NP or PA license (in the state of practice)

• 1+ year of experience in Geriatric care and Documentation review

• Excellent communication, collaboration, and teamwork skills
