Hybrid RN Care Manager - Flexible, Impactful Home Care

CareSource is seeking a Nurse Care Manager to provide care coordination, including in-home assessment, planning, facilitation, advocacy and authorization of covered plan services to meet members’ health needs while promoting quality, cost-effective outcomes.
The role involves assessing needs, authorizing services, collaborating with members and families, and coordinating across hospital, long-term care, and home settings. Strong nursing background and CRS knowledge are essential.
