Location Mesa, Arizona Department Case Mgmt-Hosp Work Shift Day Job Category Clinical Care Job Summary In the Case Manager Care Coordination position, you will provide comprehensive care coordination for patients as assigned. You will be accountable for clinical quality of Care Coordination services delivered by yourself and others, and identify and resolve barriers that hinder effective patient care. Case Managers cover units throughout the hospital and typically work in multiple areas each day. Responsibilities include conducting initial assessments, participating in daily multi‑disciplinary rounds, communicating discharge plans and updates to patients, families, and the medical team, and delegating tasks to a transitional care associate. Schedule Options Option 1 Full Time / 40 Hours Monday‑Friday 5 Eight hour shifts 7:00 am – 4:30 pm Every 3 weeks weekend rotation Holiday rotations are required Flat rate $3 per hour weekend shift differential Option 2 Part Time / 20 Hours 2 Ten hour shifts 7:00 am – 5:00 pm Every 3 weeks weekend rotation Holiday rotations are required Flat rate $3 per hour weekend shift differential Position Summary This position provides comprehensive care coordination for assigned patients. The intensity of care coordination is situational and appropriate based on patient need and payer requirements. The goal is to empower patients and families to participate fully in discharge planning. The position provides developmentally appropriate care for all ages, including the physically disabled, mentally ill, chronically ill and terminally ill, ensuring safe discharge and continuity of care. Core Functions Manages individual patients across the healthcare continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes. Acts in a leadership function with process improvement activities for populations of patients to achieve optimal outcomes. Collaboratively develops and manages interdisciplinary patient discharge plans and communicates effectively across the continuum of care. Maintains knowledge of Medicare, Medicaid and other program benefits to assist patients with discharge planning and choices; knowledge of community resources, end‑of‑life dynamics, substance abuse, neglect, and domestic violence. Establishes and promotes collaborative relationships with physicians, payers, and other health care team members; collects and communicates pertinent information to fulfill utilization and regulatory requirements. Educates internal team members on case management and managed care concepts and facilitates integration of concepts into daily practice. May supervise other staff. Operates within established procedures, consulting supervisor on unusual situations; no budgetary responsibility. Internal customers: patients, families, nursing staff, medical staff; external customers: physicians, payers, community agencies, provider networks, regulatory agencies. Minimum Qualifications RN: Must possess knowledge of case management or utilization review, usually obtained through completion of a bachelor's degree in case management or health care, and hold a current Registered Nurse license in the state of work. Social Worker: Requires a Master’s Degree in Social Work and a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW), or must obtain a license within 6 months of hire. Basic Life Support (BLS) certification is required for acute care settings. Minimum of 2 years clinical experience; demonstrate critical thinking, problem solving, communication, time‑management, and team collaboration. May require rotating call based on acute facility need. Minimum of one year Case Manager experience in an acute care hospital for Registry and Travel positions. Preferred Qualifications Certification for CCM (Certified Case Manager) preferred. Additional related education and/or experience preferred.
