Nurse/Patient Relationship: Develops and maintains a therapeutic relationship through the health care continuum. Provides direct patient care services to hospice patients. Empowers patient's caregivers by educating them on care for their loved one. Provides coordination of care, ensuring continuous assessment of each patient's and family's needs. Responsible for documentation and communication of care, medication, and observations made during care of the patient to track responsiveness and health. Responsible for providing appropriate patient/family education to help them achieve a pain‑free and comfortable status. On‑call rotation with additional pay. Pay $92,000 – $98,000 per year depending on experience. Qualifications Registered nurse with current licensure to practice professional nursing in the state. Graduate of a National League for Nursing accredited school of nursing. Maintains a current CPR certification. Current valid driver’s license. At least one year of hospice experience (preferred). Demonstrates excellent observation, problem‑solving, verbal and written communication skills, and nursing skills per competency checklist.
