Social Work Case Manager

Intermountain Health seeks a Social Work Care Manager to deliver patient-centered care management within our Healthcare & Medical Services team. In this role, you’ll complete psychosocial assessments, develop individualized care and discharge plans, coordinate community and behavioral health resources, and support patients and families through transitions of care. You’ll collaborate closely with physicians, nurses, and allied health professionals to reduce barriers, address social determinants of health, and promote safe, high-quality outcomes.
