Work Environment\n

This role allows for a flexible, remote work setting, giving you the autonomy to manage your schedule. While the majority of your work will take place from home, there may be times when in-person visits to clients are required in the community. The typical work hours are Monday to Friday, from 8:00 AM to 4:30 PM, with the possibility of occasional evenings or weekends based on client needs.

Key Responsibilities\n

• Conduct comprehensive assessments to understand the psychosocial needs of seniors, taking into account emotional, mental, and social factors.

• Collaborate with clients and their families to create individualized care plans that reflect personal goals and preferences.

• Provide emotional counseling and support during significant life transitions, such as health changes or bereavement.

• Advocate for client rights and facilitate access to necessary community resources, ensuring they receive the support they require.

• Work alongside interdisciplinary teams, including health care providers and caretakers, to monitor client needs and coordinate care effectively.

• Regularly visit clients to assess their living situations and overall well-being, ensuring safety and addressing any emerging concerns.

• Educate clients and their families about available community resources, support groups, and programs.

Patient Population / Client Focus\n

This role primarily focuses on seniors, many of whom may be facing complex challenges related to aging, such as health issues, mobility limitations, and emotional distress. Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Texas.
