Key Responsibilities\n

• Engage directly with clients both in their homes and within community settings to assess, treat, and provide support for those experiencing significant mental health challenges.

• Formulate and implement person-centered care plans that address the unique needs of each client, facilitating access to necessary services and supporting overall recovery.

• Conduct thorough assessments, including crisis evaluations, and develop appropriate interventions based on evidence-based practices.

• Maintain regular contact with clients through individual, family, and group therapy sessions, ensuring interventions are tailored to their current circumstances.

• Participate proactively in team meetings to discuss client progress and collaborate on care strategies, enhancing the quality of services provided.

• Monitor and record progress in client’s treatment plans and ensure timely documentation and compliance with professional standards and organizational policies.

• Assist in on-call coverage as needed, providing emergency support to clients and facilitating timely interventions during crises.

• Participate in quality improvement initiatives and committee work aimed at enhancing service delivery and operational effectiveness.

The social worker will primarily work with individuals in hospice care settings who face complex emotional and psychological challenges during critical stages of their lives. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

Compensation and Benefits\n

This position offers an annual salary ranging from USD 70,000 to 85,000, commensurate with experience and qualifications.
