Key Responsibilities\n

• Conduct in-depth psychosocial assessments that capture clients' mental health concerns, strengths, and available support systems, ensuring comprehensive understanding.

• Deliver individual, family, and group therapy utilizing various evidence-based treatment modalities, such as cognitive-behavioral therapy (CBT) and trauma-focused therapy.

• Develop and implement customized treatment plans with specific goals and therapeutic interventions, regularly reviewing and adjusting these plans as necessary based on client progress.

• Collaborate effectively with a multidisciplinary team, including psychiatrists and psychologists, to provide cohesive and comprehensive care for clients.

• Engage in crisis intervention and risk assessment as situations arise, ensuring clients' safety and well-being while exploring options for support.

• Facilitate psychoeducation classes and support groups to empower clients and families with knowledge about mental health and coping strategies.

• Maintain precise and timely documentation per ethical and legal standards to ensure compliance and continuity of care.

In this role, you will primarily work with individuals seeking mental health support through our Hospice Center. The patient population includes diverse individuals coping with chronic illness, end-of-life issues, and complex emotional challenges. Your expertise in working with such vulnerable clients will be critical in navigating their mental health journeys while fostering a supportive and empathetic atmosphere.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current, active state licensure as a Licensed Clinical Social Worker (LCSW) in New York.

Compensation and Benefits\n
