Key Responsibilities\n

• Coordinate care by collaborating with a multidisciplinary healthcare team, ensuring all aspects of the client’s treatment plan are synchronized for maximum effectiveness.

• Provide crisis intervention services, performing risk assessments and safety planning to safeguard the well-being of clients in acute distress.

• Advocate for clients’ rights and support them in accessing community resources, including housing, transportation, and additional healthcare services.
