We are hiring a Physical Therapist (PT) for a Home Health position providing skilled care to patients in their homes in the Colonial Park, PA area. This position is eligible for $20,000 total bonus potential , including a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $10,000 sign-on bonus paid after 30-days of employment! To receive these bonuses, you must be in good standing & meet performance expectations, and in the role and status for which you were hired!

What You’ll Do:

• \n

  Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

\n
• \n

  Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence

\n
• \n

  Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home

\n
• \n

  Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

\n
• \n

  Work independently, but never alone

\n
• \n

  Travel to patient homes within assigned territory

\n

PT Qualifications

\n

\n
• \n

  Active Physical Therapist, PT license in the state you work

\n
• \n

  One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care

\n
• \n

  Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
