Full Time Description Under supervision of Care Management Manager, works as a catalyst to promote the following: coordination of care, continuity of care, achievement of consistent and/or expected psychosocial outcomes, patient advocacy and efficient resource management. Required Qualifications Master's Degree in Social Work or related field Upon hire: Washington Clinical Independent Social Worker Associate License 3 years of Social services experience with a health care related organization; experience should include a minimum of 2 years in acute care/ care coordination. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
