SMI Adult Case Manager - Community Care Coordinator

JEWISH FAMILY AND CHILDRENS SERVICE INC is looking for an Adult SMI Case Manager in Phoenix, Arizona. This role provides ongoing case management services to members designated as Seriously Mentally Ill (SMI) who require support in behavioral healthcare.
